40 Push-Ups Reduce Heart Attack Risk by 95%? Understanding the "Surprising Numbers" from Research and Social Media

If you come across a headline on your smartphone that reads, "40 Push-ups Reduce Heart Attack Risk by 95%," you might be tempted to drop to the floor. No special equipment or expensive membership fees are needed. When told that a simple exercise can have a significant impact, it's only natural to want to give it a try.

The German media outlet AD HOC NEWS also featured this striking figure in its headline on September 13, 2026. The article uses push-ups as a starting point to discuss high-intensity interval training, building strength at home, and everyday physical activities.

However, when you return to the study that forms the basis of this article, a different picture emerges from the impression given by the headline. The question was not about the effects of prescribing push-ups as an exercise, but rather how the ability to do push-ups is linked to future health.

Here lies an important crossroads for applying health news to daily life.


The basis of the new report is a study published in 2019

According to a release from Harvard University, the research team analyzed health data from 1,104 active male firefighters in the United States. The average age was 39.6 years. Using data from 2000 to 2010, they examined the initial push-up capacity and subsequent cardiovascular disease outcomes. The study was published in February 2019 in the medical journal JAMA Network Open.

During the observation period, 37 cardiovascular disease-related outcomes were confirmed. Among the group that could perform more than 40 push-ups, only one case occurred. The university's announcement explained that the risk was 96% lower compared to groups with fewer repetitions. However, it also noted that the subjects were occupationally active men, and the results cannot be directly generalized to women or less active individuals.

In other words, just because the article was introduced in 2026 doesn't mean it was based on a new study conducted that year. In health information, it's important to distinguish between the date the news was released and the date the underlying study was published.


Three differences to verify before the "95% reduction"

When comparing the original article's headline with the primary research paper, there are three points to confirm.

First, the scope of the diseases targeted. The study's evaluation items included not only heart attacks but also cardiovascular disease-related outcomes such as coronary artery disease diagnoses. A description focused solely on "preventing 95% of heart attacks" does not accurately represent the study's content.

Second, the cutoff for the number of repetitions. In the paper's table, the group used for comparison ranged from 0–10 repetitions for the fewest group and 41 or more for the most group. While "less than 10" and "more than 40" are expressions used in the text and press releases, the table does not include exactly 40 repetitions in the highest group.

Third, the meaning of the numbers. The widely shared "96% lower" corresponds to an unadjusted incidence rate ratio of 0.04, representing a relative difference. It does not mean that the probability of onset decreased by 96 points before and after a person started doing push-ups.

Furthermore, in Table 3 of the paper, the comparison between the highest and lowest groups in Model 2, adjusted for age and BMI, showed a hazard ratio of 0.14 with a 95% confidence interval of 0.02–1.22. Since the interval crosses 1, this comparison is not statistically significant by conventional standards. Incidence rate ratios and hazard ratios are different indicators, and "96%" cannot be directly used as a preventive effect for individuals.

This result does not prove that "push-ups are meaningless." It simply means that the effect was not as simple and certain as to warrant a strong headline.


"Those who can" and "those who have started" are not the same

For example, suppose a school finds that "students who frequently use the library have better grades." This alone does not mean that increasing library visits will raise everyone's grades equally. There may be underlying factors, such as reading habits and study time, that affect both.

The same reading approach is necessary for the push-up news. The observation that "fewer illnesses were found among those who could do many push-ups" cannot be translated into a promise that "increasing the number of repetitions will prevent illness by that proportion."

This difference is not a matter of fine wording. In the former case, the number of push-ups can serve as a clue to assessing physical fitness. In the latter case, reaching a specific number might be perceived as a method of treatment or prevention. It can change the reader's subsequent actions.

There is convenience in the numbers from fitness tests. You can become specifically aware of your changes, such as doing more than last time or feeling less fatigued. However, it's important to distinguish between a convenient benchmark and a test for determining future illnesses.


On social media, voices of surprise and "Is that causation?"

This study has been discussed online since its release. The following introduces reactions to the research and reports posted on Reddit in 2019 and 2020, not direct responses to the 2026 AD HOC NEWS article. The following is a summary of actual English posts in Japanese and does not represent a survey of the overall opinion distribution on social media.

 

In the 2019 science community "r/science," user badatmathmajor pointed out not to confuse the association between push-up proficiency and cardiovascular disease with the causal effect of push-ups themselves.

Additionally, Dizzy_Slip questioned whether there might be people with high endurance who still couldn't do 40 push-ups. Seven_seven asked what to consider if the limit on repetitions is due to wrist issues.

There were also humorous reactions. Lmbb20 exaggerated their anxiety by posting, "I can only do 35," highlighting how numbers can easily be perceived as pass/fail boundaries.

In the 2020 bodyweight training community "r/bodyweightfitness," International_Legacy expressed doubts about reports deriving causation from the association between push-up repetitions and heart disease. They suggested considering the possibility that lifestyle habits, including exercise and diet, might affect both.

These posts are not medical evidence themselves. However, they teach us what to question when reading research. "Does it apply to me?" "What is being measured?" "Does it reveal the effects of changing behavior?" Even without knowing technical terms, these three questions can be held.


No need to blame yourself for not being able to do 40

The tricky part of this topic is that the more straightforward the numbers, the more they tend to be linked to personal evaluation. Those who can do 40 feel reassured, while those who can't feel discouraged. However, the cutoff used in the study to categorize people does not automatically become a passing line in everyday life.

The wrist issue mentioned on social media brings this problem closer to home. Whether the reason for stopping push-ups was overall fatigue or joint pain, the same number of repetitions should mean different things. Recording only the number of repetitions erases such context.

When keeping a record of your exercise, it might be good to note whether it was done without strain, if there was any pain, and if it was done in the same manner, in addition to the numbers. This is not for self-diagnosing illnesses but for reflecting on daily exercise.

Rather than comparing with social media records, it's easier to consider goals by observing changes in yourself under the same conditions. Increasing repetitions is not the only progress. Maintaining a calm form or incorporating exercise time into your life are also important achievements for continuity.


If applying from today, focus on exercise combinations rather than repetitions

So, if you were to incorporate this news into your life, what should be the goal?

The broader guideline for health improvement is the WHO's recommendations on physical activity. For adults, the basics are 150–300 minutes of moderate-intensity aerobic activity per week, or 75–150 minutes of high-intensity activity, or a combination of both. Activities to strengthen major muscle groups are also recommended at least two days a week.

Enjoying push-ups and making time for walking can coexist. For example, if you walk briskly for 30 minutes on weekdays, it totals 150 minutes, which corresponds to moderate intensity. However, this is an example of time allocation and not a quota to be achieved all at once, disregarding current fitness levels. WHO also indicates that even a small amount of physical activity is better than none.

Regarding strength training, the Mayo Clinic emphasizes proper form, not holding your breath, and taking rest for recovery. If you can't maintain form, reduce the load or repetitions, and stop exercises that cause pain. If you're starting for the first time, having a professional check your movements is an option.

The goal of "quickly achieving 40 repetitions" might be motivating for exercise enthusiasts. However, what health articles aim to convey is not that efforts are wasted if you don't reach that number. Walking, using muscles, resting—arranging each in a sustainable way makes it easier to incorporate into daily actions.


The value of health news lies in the step after the surprise

The figure "95% reduction" has enough power to capture many people's attention. However, there should be time to think between the moment of surprise upon reading the headline and deciding what to start.

What year was the study? Who was studied? Was the effect of exercise tested, or was the relationship between ability and health observed? Even after these confirmations, there's no need to lose interest in physical activity.

The number of push-ups can be a trigger to focus on your physical fitness. However, one number cannot dictate future health. After reading this article, perhaps the goal should be to find one exercise you can continue, rather than rushing to make your record 40.
