"Whether or not I will develop dementia in the future is something to think about when I'm older."

Many people might think this way.

However, recent studies have repeatedly pointed out that one of the crucial periods affecting brain health in old age is actually the "middle age" of the 40s, 50s, and 60s.

A large-scale long-term study in the United States, published in 2026, illustrated this impact with very clear numbers.

The number that drew attention was "12.6 years."

It was found that there was an average difference of 12.6 years in the "period of living without dementia" between people who did not have the three vascular risk factors of hypertension, diabetes, and smoking in middle age and those who had all three.

It's not just about "what percentage higher the risk of dementia is."

This study has a significant impact because it shows the potential for a substantial difference in the time one can live while maintaining judgment and memory in the later stages of life.


Tracking 12,409 people for about 26 years

The data used in the study comes from the "Atherosclerosis Risk in Communities (ARIC)" study, which has been ongoing in the United States for a long time.

The analysis focused on 12,409 people.

The average age of participants was about 56, and they were not suffering from dementia at the age of 55.

The research team examined whether participants had the following three risk factors in middle age.

"Hypertension"

"Diabetes"

"Current smoking habits"

They were then tracked over a very long period, with a median of about 26.3 years.

During the tracking period, 3,008 people developed dementia.

Meanwhile, 5,238 people died before developing dementia.

Properly considering this "death before developing dementia" is an important point in viewing this study.

Simply looking at "how many people eventually developed dementia" would mean that those who died early had fewer opportunities to develop dementia. Therefore, the research team considered both death and dementia and analyzed "how many years one could live without dementia between the ages of 55 and 95."


A "12.6-year" difference between 0 and 3 risk factors

The results were very clear.

The average period spent without dementia after age 55 was:

30.1 years for those with 0 risk factors.

26.3 years for those with 1 risk factor.

21.0 years for those with 2 risk factors.

17.5 years for those with all 3 risk factors.

In other words, there was a difference of 12.6 years between those without hypertension, diabetes, and smoking and those with all three.

Furthermore, those with all three risk factors had about 2.69 times the hazard of developing dementia compared to those without any risk factors.

Even greater was the "risk of dying before developing dementia," which reached about 5.61 times.

It is important to note that this does not mean "eliminating the three risks will definitely delay the onset of dementia by 12.6 years for everyone."

This study was not a clinical trial randomly assigning medication or lifestyle improvements but rather a study observing people's health over a long period.

Therefore, what was shown is a strong "association."

At the start of the study, those with more risks also showed differences in other characteristics such as BMI, exercise amount, education history, and stroke history.

Nevertheless, the fact that the period one could live without dementia gradually shortened as risk factors increased is a result that cannot be ignored when considering the importance of vascular management in middle age.


Why "blood vessels" are related to brain health

When hearing the word dementia, many people might first imagine "aging of brain cells."

However, the brain is also an organ that requires a very large amount of blood.

Therefore, it is important not only to hastily take measures after becoming elderly but also to manage health decades in advance.

For example, for a 50-year-old, 80 years old is 30 years away.

"30 years" is a very long time.

Even if the differences in daily or weekly lifestyle habits seem small, if they accumulate over decades, they can lead to significant differences in blood pressure, blood sugar, weight, and vascular condition.

What this study shows is that "while there are factors that cannot be changed, there are also factors that can potentially be changed."


WHO also focuses on "modifiable risks"

This direction is not unique to this study.

The World Health Organization (WHO) updated its guidelines on reducing the risk of cognitive decline and dementia in July 2026, stating that up to about 45% of dementia risk is associated with modifiable factors.

These include not only smoking, lack of physical activity, hypertension, and diabetes but also excessive alcohol consumption, social isolation, air pollution, high cholesterol, and hearing problems.

In other words, the "hypertension, diabetes, smoking" highlighted in this study are not all there is to dementia prevention.

However, they are also important factors common to the prevention of myocardial infarction and stroke, and one measure may lead to a reduction in the risk of multiple diseases.

Thinking "what's good for the heart is also good for the brain" makes it easier to understand.


Three points to consider starting today

So, what should we learn from this study?

The first is to know your blood pressure.

Hypertension often has few noticeable symptoms.

"I'm fine because I don't feel unwell" is not enough; it needs to be measured to know.

The second is to regularly check your blood sugar levels.

Diabetes may not show strong symptoms in its early stages.

It is important to undergo health check-ups and consult with a doctor about blood sugar levels and HbA1c as needed.

The third is to consider quitting smoking if you smoke.

Anti-smoking measures are not solely for the purpose of preventing dementia.

They are related to many risks, including cardiovascular disease, stroke, lung disease, and cancer, so the health benefits of quitting smoking are extensive.

It is also important not to focus solely on these three points.

Engage in regular physical activity.

Aim for a balanced diet.

Maintain an appropriate weight.

Continue treatment for hypertension or diabetes if necessary.

Maintain social connections.

Do not neglect hearing loss.

Combining these multiple efforts leads to long-term health.

It should be noted that on social media and in comment sections, there are sometimes opinions that certain supplements or foods are "effective in preventing dementia."

However, personal anecdotes and medical evidence are different things.

The latest WHO guidelines do not recommend the uniform intake of vitamin B, E, omega-3, or multivitamins and minerals for dementia prevention in people who do not have confirmed deficiencies.

Rather than simple methods like "drink this to prevent it," basic health management such as blood pressure, blood sugar, smoking, exercise, and diet is far more important at this point.


Thinking about "time spent healthily" rather than "whether one will develop dementia"

Perhaps the most striking aspect of this study is that it showed not only the "probability of developing dementia" but also the "time one can live without dementia."

Even if life expectancy increases, it does not necessarily mean all of it will be spent healthily.

Therefore, what will become important in the future is not just life expectancy but "healthy life expectancy" or "the period one can live while maintaining cognitive function."

If one can live 30.1 years from age 55, it corresponds to living without dementia until the mid-80s by simple calculation.

On the other hand, 17.5 years would mean the early 70s.

Of course, this is an average for the group and not a prediction of an individual's future.

Nevertheless, the 12.6-year difference is significant enough as a reason not to postpone health management in middle age.

When thinking about dementia measures, we tend to focus on "what to do for the brain."

However, what this study shows is a more fundamental approach.

Manage blood pressure.

Manage blood sugar.

Do not smoke.

Be physically active.

Maintain a healthy diet.

In other words, rather than treating the brain as special, keeping the whole body healthy ultimately protects the brain.

Dementia is not a disease that can be completely controlled.

There are factors like genetics and aging that cannot be changed.

However, "there are things that cannot be changed" is not the same as "nothing can be done."

The state of blood vessels in one's 40s, 50s, and 60s may be deeply related to the time one can live life on their own terms later on.

The day to start doing something for the brain
